ADMISSION CRITERIA



The Dubois-Douglas Centres accepts persons with disabilities for residential services based on the following criteria, without regard to race, color,national origin,religious preference,sexual orientation other disabling conditions, as vacancies exist:

**Must be a developmentally disabled and/ or mentally ill person as defined by the Illinois Mental Health & Developmental Disability code;

**Must be 18 years or older, unless approved by DMHDD;

**Or guardians, must agree to participate in development and implementation of their IPP.

**Must meet criteria specific to an individual program option sit;

**Must have had no incidents of physical affression for six months prior referal;

**Must be free from communicable diseases(except HIV or HBV) and have a DDC medical examination report completed within 30 days prior to admission, if residing in a group setting;

**Must be a recipient of benefits equal to the maximum SSI benefit level, or present as evidence of recent award letter of application for SSI/SSDI, OR PAY STUB.
